  1. What formal Work Health and Safety (WHS) risk assessments have been conducted for GIO Stadium in the past five years specifically covering risks to (a) players, (b) coaches, (c) match officials and (d) operational staff.
  2. Do WHS standards applied at GIO Stadium meet the same requirements that would apply to a public servant workplace under ACT Government policy; if there are differences, what are they.
  3. Has SafeWork ACT, or equivalent regulator, been notified of any incidents and have any investigation or improvement notice been issued.
  4. What formal hazard identification processes are in place for temporary occupants, such as visiting teams, using enclosed or elevated facilities such as coaching boxes.
  5. Who holds ultimate responsibility for building compliance and WHS safety at GIO Stadium, including glazing compliance, and how is that accountability enforced.
  6. Does the Government require major public sporting facilities to meet the National Construction Code and contemporary Australian Standards, including AS 1288, to the same level as new buildings, or are legacy exemptions applied.
  7. Will the Government commit to a full independent building condition and safety audit of GIO Stadium, including (a) glazing, (b) structural integrity, (c) water ingress and (d) WHS risks, given the recent incident at GIO Stadium where a coach sustained lacerations after a glazing panel shattered.
