(Safety Incident and Compliance with AS 1288 (Glass in Buildings))
In relation to the recent incident at GIO Stadium where a coach sustained lacerations after a glazing panel shattered, (a) what type of glass was installed in the coaching box, including (i) specification, (ii) thickness, (iii) whether it was laminated or toughened safety glass and (iv) does that installation comply with AS 1288:2021, (b) was the glazing in the coaching box classified as “subject to human impact” or “overhead / elevated glazing” under AS 1288; if so, what compliance pathway was used to ensure safety, (c) what engineering assessment has been undertaken to confirm that glazing in elevated positions above players, officials or spectators cannot result in falling glass fragments in the event of failure, given AS 1288 addresses risks associated with human impact and overhead glazing, (d) does the Government require laminated safety glass, rather than monolithic toughened glass, in elevated or occupied areas of GIO Stadium to mitigate the risk of falling shards; if not, why not, (e) has any heat-soaking, containment system or barrier design been applied to glazing at height to prevent spontaneous breakage risks associated with toughened glass installations, (f) when was the last independent certification or audit of glazing compliance at GIO Stadium against AS 1288 undertaken, (g) had any defects or operational issues with the relevant window or glazing system been identified prior to the incident; if so, when and what action was taken and (h) how many glazing-related defects or failures have been recorded at GIO Stadium over the past 10 years.
